Step-by-Step: How to Apply

Step 1: Complete the Family Intake Form

Parents start by filling out our short online intake form.
This helps us understand your child’s needs and what type of support or services you are seeking.

Step 2: Provide Supporting Information

Families may be asked to provide:

• Recent evaluations or school/medical documents

• Insurance status or denial letter (if applying for therapy scholarships)

• Basic financial information (only if required for certain programs)

Step 3: Review & Eligibility Determination

Our team reviews each application to determine:

• Eligibility for services

• Available openings or funding

• Recommended programs or resources

Families will receive a decision within 10-15 business days.
